I understand Tara Becker School of Dance accepts no responsibility for injury or loss caused during classes or whilst students/participants are at or near the dance studio. You are responsible for ensuring that your child is physically and medically fit for the class. (This must be agreed to in order to participate in any studio activity)

Medical Emergencies:

In any event of an accident or emergency I hereby authorize my child to be transported to a hospital for medical treatment and I hold Tara Becker School of Dance and it's representatives harmless in the execution of such. Additionally, I hereby agree to individually provide for all medical expenses which may be incurred by me or my child(ren) as a result of any injury sustained while participating at or for Tara Becker School of Dance. (This must be agreed to in order to participate in any studio activity OR other instructions must be given in writing to be added to your child's file).

Photography Release

I understand Tara Becker School of Dance may take photos of my child at the studio, or at other dance school events and video footage in class and/or at the end of year show. I, as the parent or guardian will be contacted if these photos or footage will be made public for advertsing purposes, and I understand I have the option to decline.

Photography Release for 'In-House' display

I understand Tara Becker School of Dance may use photographs of my child (in a group/class photo) inside the studio for decoration unless a letter is forwarded to the school outlining that you as the parent/guardian to not wish for this to happen. (This must be agreed to OR a declining letter must be forwarded in order to participate in any studio activity)
